How to apply for a cloth nappy subsidy
The subsidised cloth nappy purchase is operated via a voucher system. You will receive a voucher when you have completed the application form.
What do you need to do?
You can get an application form from the Customer Services Agency, Plaza Level, Civic Centre or you can download it here.
Cloth nappy application form
You will need to provide the following documentation with your application:
* Photo ID - either a passport or driver's licence
* Verification of your address - bank statement or a bill
* Baby's birth certificate or confirmation from health care provider that mother is in her last trimester (6-9 months pregnant).
You must read and agree to our terms and conditions of obtaining a cloth nappy purchase voucher by signing and dating your application form.
Terms and Conditions
Agreement to these terms and conditions is a mandatory requirement for obtaining the cloth nappy subsidy.
* Only one $20 cloth nappy voucher issued per child.
* Each voucher can be used only once at a participating supplier and must not be duplicated. Infringement will result in you being charged for any additional cloth nappy purchases.
* The voucher must be claimed within the valid time period - last trimester of pregnancy through to baby's first nine month - by primary parent / caregiver.
* You must be a Dunedin resident to apply. Please provide: photo id, baby's birth certificate, and verification of address details.
* Your contact details will be kept on file under strict confidentiality. You will receive a follow up phone call from a DCC representative within 3-6 months of your cloth nappy purchase, when you will be asked a few questions about your cloth nappy usage.
